? putts ....question

I have a golf question, trivial, but I'd like to know the answer.
It's about the number of putts on the green.
Scenario: Player hits onto the green. He putts but puts it off the green, in deeper grass, lets say, and then wedges out onto the green again. He then 2 putts in.
Question: Did he 4 putt the green or 3 putt?
What if he didn't wedge out but used his putter from the higher grass off the green? What if you land on the fringe around the green and choose to putt?
When does the # on your putt strokes start? All after you initially land on the green, or just all hit from the green?

Let me try to tackle this...

If you are on the green and putt off, like the 1st scenario, that would be considered a 4 putt, or however many it takes you to get it in...even though there is a chip in there. Makes no difference if he chips it or putts it from the rough.....they are all considered putts once you are on the surface of the green.

I heard an announcer state awhile ago that the given scenerio would only be a 3-putt. Cant recall who it was and I cant find anywhere that gives us an answer. After hearing that i've always taken that as the correct answer. Will be interesting to find out which is fact given that it happens to me all the time.
