A lot of the road work is done at night, but you are right, it is a pain in the ass and with the amount of crazy drivers out here (usually with California plates) it makes it highly stressful especially in the morning and evening commutes with the bright ball of red in your eyes this time of year!!
On a side note....to those who have sent me words of encouragement or posted notes of sympathy for my daughter....we got a text from her last Monday asking to come home to detox and get off heroin....well after a week of white knuckling it and some rough days I can say she is 8 days sober and although she has a lifetime of cravings and temptations ahead of her, she is clean and that is all I have asked for a long time....now the work begins

Pics from earlier this week. This is the result of back to back plays. So close...
This is the third night in a row at work. It's been fairly uneventful, but I'll post a few things. Two nights ago, one of my coworkers comes to me for help with an IV. IVs are my thing, and actually my previous job I carried a beeper and phone and started difficult ones all over the hospital. I go in the room, and the patient has half of his head shaved with a stapled incision. I asked him what happened, and he said he fell into the side of a building. He denied alcohol use when I asked but readily admitted that he "muscled". This means he shoots heroin into his muscles instead of his veins because he has ruined all of his veins from prior intravenous heroin use. He was vomiting and withdrawing hard (vomiting, profuse sweating, severe abdominal pain, and agitation). Starting an IV on a dude like this is about impossible. I was able to start one, and he received IV Dilaudid, Ativan, and Zofran. That helped him a lot, but when I came in the following night, I was told he left AMAHe couldn't stand the hospital confinement and treatment, so he left against medical advice to hit the streets again. We see patients like this all the time. Very difficult to care for, and they leave you to wonder why you even bother?
Another patient has just been diagnosed with stage IV liver cancer. She is very nice but miserable because she has a terrible cough. I called her doctor who is a complete moron. I can't stand doctors that suck and don't care. They are lower to me than criminals, and I see things that need to be done that they will not do. I want to just tell the patient their doctor blows and to get somebody else, but of course that would probably get me fired. Anyway, I called this idiot doctor and told her what I needed (cough medicine and tachycardia medicine). She was fine with the heart rate medicine but wanted me to give some OTC bs robitussin that will not work. I told her that was not good enough, she got pissed, I stood my ground, and she finally said, "fine! Do whatever u want!!" Cool; I ordered the good cough medicine, the patient stopped coughing, and she slept. Her heart rate went from 130 to 70.
If you ever have to be hospitalized, pray that your doctor gives a damn about you because so many do not. It's so horrible the medical care that I see sometimes. Just beyond criminal imo. You want an aggressive nurse that isn't scared of doctors that will get stuff done that you need because otherwise, you might be out of luck.

So tonight is my 5th out of 6 at work. I got floated to a crappy floor that is super disorganized and kind of the dumping ground of the hospital meaning this is where there are a lot of long term patients that can't be placed anywhere else for discharge. I have one patient who is 50 and refuses to get out of bed. He is Bedfast and incontinent by choice. What a life; being in a hospital bed 24/7 pooping and peeing on yourself when you can walk under your own power. Some things I will never understand.
I have another patient who is like a giant blob. She reminds me of that ghost from Ghostbusters. She is only 69 years old, and she is so lazy that she also lays there and poops and pees on herself when she can walk. She just wants her benzos and narcs. Her daughter who is 42 stays in the room with her, and she is drugged up too. She actually pissed all over herself and her pull out bed in the room :facepalm:
She actually called requesting we change her bed when she's not even the patient!! Ridiculous! She is supposedly homeless and says she is a domestic violence survivor. Her mother has been here for 6 months!! Go home already!!
Another patient is here for abdominal pain. She is 24, and all of her tests have come back negative except for an ovarian cyst. She is getting Dilaudid every 3 hours for an ovarian cyst!!! Really?? All women have ovarian cysts. Give me a break. So absurd!
Another patient is here because he doesn't take care of his diabetes. He is actually refusing insulin here in the hospital. I'm sure he will get discharged and be right back next week for the same problem. Ridiculous as well.
I estimate that 75% or more of hospitalized patients are in the hospital secondary to life choices. Most of my patients are here because do stupid things or they want to be here.

Thanks, hedgehog! Kneifl, my opinion on your question is this. I think doctors have a lot on their plates, and because of this, they start taking shortcuts. Before all the technology, they had to assess, diagnose, and treat using their skills. Now, many of them just order a litany of tests that will take the thinking out of it. Many doctors never even lay a hand on the patient anymore. I think that cutting corners to save time has become epidemic and commonplace now. A true thinking diagnosing physician is becoming hard to find.
A nurse with experience will absolutely know what their patient needs and what is wrong with his patient and what needs to to be done to treat and manage care. It's a matter of getting the doctor to order and do these things.
You rarely if ever see the doctor round with The nurse. This used to be the standard. Now it is more standard to see the doctor sneak on and off the unit or do their rounds thru cyberspace and not even see the patient at all! It is truly pathetic and a sad state the things I see. I hear patients every shift talk about never seeing their doctors, and it's true! I also see no accountability for the actions of negligent and just plain bad physicians.
Looks like a fair amount of your patients are good candidates for the Free Soylent Green factory tour...
