Foot odor:

So, I have a patient tonight who is very nice. She's actually a nurse, and she came to the ER last week with severe abdominal pain. After being evaluated, the tests showed that she had free air in her abdomen. This is bad, and it means she has to have emergency surgery. She had a perforated viscous, and she is recovering well now.

The problem tonight is that her 25 year old son came to visit and stay the night. He's a nice enough kid, and he doesn't ask for or need anything, but when he took off his shoes, holy hell!! The odor that I smelled nearly made me gag. How can you be an able bodied young man and not know or worse not care that your feet smell this bad?

My solution was to shut the patient's door. This turned out to be ok for a while, but when I had to go back in the room, the smell would gag a maggot. I exited fast and reshot the door. I just saw the x-Ray tech go in the room. He opened the door, got a face full of the stench, and he backed out. He looked at me and mouthed the words :wtf:
I pointed at my feet, he understood the gesture, and he ventured back in. Afterwards he asked me about it, and I told him I didn't understand how it could be that bad and him not know it.

Platinum card:

I now have a Platinum card with the Ceasars/HarrAhs properties. I'm not sure what all I'll get if anything, but I do know there are some two for one shows offered, and it looks like I can stay for free during the week at two more properties than before (Paris and PH). We will probably go to the Paris to stay on Tuesday because we really enjoy getting rooms at the casino hotels.

Last thing, Jack was, still is, and is always going to be an animal when it comes to gambling. I have such a great time when he comes out. Smurphy will be out here in a few weeks, and he's also an animal but in different ways haha. Enjoyed hanging out with Feelgood as well, and we talked about getting together more often since we both are residents.

Work stuff and maggots:

Just a snapshot of some of the patients I take care of in the hospital. The first guy is 44. He was at rehab in an effort to get off narcotics. Unsuccessful, and out of control, he was shipped to the hospital. He presents as acting very crazy. He is thrashing in the bed but apologetic for it. He is also bipolar, and he says if he doesn't move and jerk like this, he fears his bones will break and this behavior prevents that :facepalm: He needs a new IV, but he's so out of control I can't do that. He has nothing ordered for pain or anxiety yet his diagnosis is withdrawal symptoms. I guess the idiot doctor expects me to wiggle my nose or perhaps entertain him with my good looks. I page the doctor with this message, "need pain and anxiety meds. Patient is out of control." I get the meds, give them, and he calms a bit. I asked him what he likes to be called, and he says "Pastor Paul". He said he went to the seminary 8 years. He has a congregation, but it's not the usual church. He quotes me the scripture Genesis 9:3. "Every moving thing that liveth shall be meat for you; even as the green herb have I given you all things." This guy grows and sells his own pot; I guess he learned that in seminary? Anyway, he starts acting like a complete jackass again, and I called him Pastor Pot. He gave me an incredulous look and asked how I would like it if he made fun of my college degree and demeaned it with a name. I told him I wasn't making fun of him but rather trying to make my terrible situation of caring for him better for us both. He laughed, apologized for acting like an idiot, and he improved. I hope I never see pastor pot again. Psych patients wear me out.

Patient number two is homeless and an alcoholic. His record says, "he was found down on the ground at a gas station surrounded by beer cans." This guy is pretty easy though because he is very content to eat, sleep, and get his narcotics and anxiety medications. Most homeless patients are easy albeit stinky. If they are able, I make them get straight in the shower as soon as I get them. U just can't imagine how dirty they are. It's a terrible existence living on the street.

Patient number three is an assault victim. Per his record, "patient was hit over the head with a wine bottle. He had gauze on his head, and when removed three lacerations were noted to the top of his head. He is confused and only oriented to himself. When undressed, there is noted to be a scrunchie tied around his penis three times that had to be cut off. His genitals were noted to be very pale before the scrunchie removal and noted to turn a bright red after it was removed. " It turns out that his girlfriend did the damage, but the scrunchie origin was never determined. I'm not sure if it was a homemade cock ring or if the girlfriend did it after he was unconscious in attempts to harm his penis. He was too out of it to tell me, so it will probably always be a mystery.

The last guy is also homeless, but he denies it. He was found confused at the airport and intoxicated. He claims to drink a 6 pack a day. He is skinny, has a full beard, and he is 64 years old. I figured he was a veteran, so I asked him, and he said he served 3 tours in Vietnam. I thanked him for his service, and even though he was filthy dirty with a horrible foot wound and frequent incontinent diarrhea, I really liked him. He never complained and rarely asked for anything. He told me his foot had been hurting for weeks. He said that his foot wound had maggots in it which was confirmed by the chart. He said he would wash them out at night, go to bed, then have a sock full of them again in the morning. Can you imagine having a sore with maggots in it? As gross as that sounds, it probably is what will prevent him from losing his leg because the maggots actually keep a wound pretty clean. Sadly, I see homeless veterans in bad shape routinely. I wish I could do more for them. More later, and count your blessings!

I'm in the process of investigating that. When I find it, I'm going to post in here.

Just found the thread, I find the patient stories the most interesting. Heroine.. What a drug this is... It's everywhere and more and more becoming addicted. Glad you are doing your best to help out those in need.

So, quite an interesting last hour. I see a nurse running down the hall, and she is trying to page the float nurse. I immediately followed and found her patient barely breathing, what I would call agonal breaths. I looked at her heart monitor, and it showed bradycardia with a rate of 34. This is a witnessed respiratory arrest which causes cardiac arrest and then of course death. I ran for the code cart, we called a code blue, and commenced with resuscitation efforts. Code blues are ugly and sometimes even haunting. This one turned pretty bloody because the patient started bleeding from her mouth for some reason (probably ruptured esophageal varicies from alcoholism). Anyway, we got a rhythm back on her, intubated, and sent to a medical ICU. I didn't feel any ribs breaking when I did cpr, but that's very common too. I doubt this lady will make it because of all the things wrong with her, but I hope she does. I took a shot of my scrubs and the blood splatter.

Tonight at work I have a patient who was beaten almost to death by his mom's boyfriend :rolleyes: he has a head injury and brain bleed, and he has been here 10 days. He is much better, but he is considered a medical hold because he is not yet quite competent. He wants to leave, so now it's my job to coerce him to staying in the hospital. He did not have to have surgery, and the treatment is to give him steroids and let the bleed reabaorb over time. He feels as if he is being held against his will for no reason. I explained to him how fortunate he was to not be wearing a helmet. I told him I know he has seen guys with head injuries so bad they have to have surgery and then have to wear a helmet. That seemed to scare him straight a little, but he wanted to reassure me he could go home tomorrow (something I can't do of course). I finally bargained with him and our agreement is that he gets a CT scan tomorrow and then home if it's better. It's not a bargain I can completely keep because I'm not the doctor, but by then I'll be gone and somebody else will have to deal with him.

If he decides he has had enough and starts walking out, I have to call security, he will be detained, and then he will be restrained. I don't want that to happen, and I really hate situations like this. It's really a no win deal for me. Hopefully he will be chill. Typical stuff I have to deal with at work.

I love this casino. It's so nice inside and not smoky, and the staff all seem nice. Their comp system is stupid and I can't make sense of it, but oh well. I'm going to have to read about it but the slots/vp, table games, and poker room are all separate? Weird...

I started off a $40 loser on VP and then a $140 loser playing poker. My poker game was soft, but I couldn't get any traction. The last hand, I had too pair but the guy got runner runner for a straight. Probably over $200 in the pot. I went on to win at VP just playing quarters. The VP is great there. I even found DDB 9/7 at the bar, and there are several banks of progressives. I played several sessions at blackjack and won at all of them putting me up around $300 on the night. It's been a drought, so I was happy to win and cut the night short (I'm used to staying out until 4 or 5 am).

Driving home, an asshole state trooper pulled up beside me and flashed his spotlight in my car?? :wtf: idk what that was about, but he drove on. That's dangerous to blow somebody up like that when they are driving.

This is a douchebag with a tiny dog I saw driving earlier. He was darting in and out of traffic in his convertible with his dog :rolleyes:

Thanks for all the nice comments. I feel I'm so blessed to be able to work as a nurse. It's my privilege, and for the most part I love people and helping them out when I can.

Out this morning early to do errands, and then met a friend from home who I just found out lives here. Saw this guy on the way. I can't stand these scooters.

I see trauma patients every week who get hit while riding them; it's almost epidemic, and they are so unsafe.

We had lunch and my Platinum card paid for it, so a freebie! He had been up all night, so he left, and I wandered over to the Mirage. The 11:00 tourney was going, and the entry fee was $65. That mandated I play. I played well for over an hour then got it all in preflop with AK vs AQ. The queen of course hits the flop, I don't improve, and busto. I went over to craps and played dont for a small profit then played with a pretty young redhead (Jack, u would have loved her) who proceded to roll 3 points. I cashed out a $175 profit.

I lost $75 screwing around with roulette and DDB VP, so I left and went to Casino Royale. Casino Royale caters to lower rollers, but it's very fun. I was up $100 playing single deck BJ when I got a 10 vs a dealer 6 with a $50 bet. I doubled down of course and got a 10. The dealer then goes on to hit a 5 card 21 :facepalm: so I left even instead of being up $200.

I went a few tables over to free bet BJ and netted a $100 profit before leaving. I went to Harrah's next and made another $100 playing BJ. That was nice, and I'm getting ready to go home now to take B to work. I'll probably hit it somewhere later tonight, but I'm up for the day and happy. I have Miami over tonight, so that will be nice if it wins.

I got to make good use of my Platinum card because the cage line at Harrah's was long, so now I'm one of those dick line cutters haha!
